If you have created, have worked on, or know of a film, radio, or television production with an initial public release between January 1, 2005, and December 31, 2005, that offers a respectful depiction of people with mental health problems, please submit a nomination.
To make a nomination, tell us where and when you saw or heard the production you wish to nominate. Please indicate the following information:
- Production medium:
- Film, TV, or radio
- Type of production:
- Series, feature, episode, or documentary
- Title of production
- Date and time of initial public release:
- For TV and radio: Date and time the program aired
- For film: Date you saw the film
- Outlet of initial public release:
- For TV: Name of broadcast network or station, or cable television channel or network
- For radio: Call letters of station or name of radio network
- For film: Name of film
- For all: City and State
- Name(s) of writer(s) or producer(s)
- Episode name or number (if applicable)
- Reason for nomination (you may attach one additional page if you need more room)
- Your name and contact information (OPTIONAL—you may nominate anonymously)
- If possible: Your name, address, phone, and/or e-mail
When nominating, please be as specific as possible. If you know the name of the writer(s) and producer(s), please include them. If possible, please send a CD, DVD, or videocasette of the production you are nominating. At a minimum, please include the production’s title and date, time, and outlet (TV network or channel, cable network or channel, radio station, movie studio, etc.) of initial public release in order for us to be able to consider your nomination.

Nominate Consumers
Mental Health consumers throughout the United States have played a vital role in raising awareness and understanding of mental health problems, and in reducing the stigma and discrimination associated with them. Their work helps to ensure that people are able to access services and supports that assist recovery. In addition, through their leadership and advocacy, they demonstrate that recovery is real and that people with mental health problems are valuable, contributing members of their schools, workplaces, and communities.
For the purposes of the Voice Awards, the term "mental health consumers" is defined broadly as individuals who have received or are receiving mental health services for a mental health problem.
If you know anyone who has made a particular contribution in the following areas, please consider nominating them for a 2006 Voice Award. Nominees should be mental health consumers who have made outstanding contributions in all of the following areas (self-nominations are welcome):
- Led efforts to reduce the discrimination and stigma associated with mental health problems
- Personally demonstrated that recovery is real and possible
- Made a positive impact on their community, workplace, and/or school
Additional consideration will be given to nominees who have made a positive impact in special populations such as older adults, racially and ethnically diverse groups, and rural communities.
To make a nomination, please submit a short essay that includes the following:
- Consumer’s name and contact information
- Your name and contact information
- A description (500 word maximum) of the contributions of this person as they relate to the three criteria listed above
Nominations are free and open to anyone, and self-nominations are welcome. All nominations must be received by April 21, 2006, and should be sent by electronic mail to voiceawards@vancomm.com. If you do not have access to the Internet through a personal or business computer, keep in mind that most communities have libraries with free Internet access. While e-mail entries are strongly encouraged, if you do not have access to a computer, paper entries can be mailed to:
